Athletic Clearance is required for students participating in Upper School Athletics.
Required forms
- FHSAA EL2 (Preparticipation Physical Evaluation): All new physicals must be completed on the FHSAA EL2 form, and only page 4 should be uploaded to Holy Trinity’s Online Athletic Clearance. The physical exam must be documented on the EL2 form for athletics; no other forms will be accepted.
